Where do bad MD's come from?

Some people are good in an academic setting but have zero bedside manner, business sense or ability to operate under pressure. There may be some scattered cases of dubious degrees or doctors who are battling addiction problems or mental illness or personal issues that prevent them from doing their job (then again, some mental illness may have no impact on their job), but I think the majority are just people who cannot handle the job.
